How does the
membership work?
The
CMTC Wellness Membership option allows members to
choose their desired health maintenance level by
signing to a minimum of six month membership
commitment with Carrico Muscle Therapy Center or
CMTC. After the first six month commitment
elapses, CMTC will renew the membership
commitment automatically for consecutives six
month periods.
Additional
treatment sessions may be purchased by a member
at a membership rate.
The
chosen Wellness Membership monthly fee will be
automatically deducted based on direct bank
withdrawn.
Family
Share
As
a member, you can add one other family member who
resides with you to enjoy your membership rates.
Care
to Share
CMTC
allows members to share a current unused
treatment session with guests if a member is
unable to use a monthly treatment session.
Monthly
treatment sessions cannot be accumulated.
Cancellation
Policies
A
member may cancel his or her Membership Agreement
during the initial term of the membership if a
member’s permanent residence changes to a
location more than 30 miles from the clinic.
Cancellations must be accompanied by written
proof (i.e. driver's license, utility bill, car
insurance, etc.)
Cancellation during auto-renewal term
A
member may cancel his or her membership upon
30-day advance written notice to
CMTC clinic.
Once
a member has been approved for cancellation him
or her will be relieved from making payment for
membership dues.
